We thank Drs Jilma and Jilma-Stohlawetz for their interesting comments on our study on MCP-1 lowering effects of HRT in postmenopausal women [1]. Their finding of lower MCP-1 levels in 38 premenopausal women compared to 58 men [2] confirmed data from Inadera et al. taken from a cohort of 405 Japanese healthy subjects [3]. Lower levels of MCP-1, depending on the phase in the menstrual cycle, were observed by Jilma et al. in a group of 18 women. This is of interest, but does not establish causality. Therefore, in our opinion, concluding "female sex hormones decrease MCP-1 levels" is not warranted based on data presented by Jilma et al. In our intervention study, in an appropiate population, we established causality: Hormone replacement therapy decreased levels of MCP-1. In hindsight, the data from Jilma et al. can be considered to be in line with our conclusion. Indeed, it will be interesting to see whether a synergistic effect of ACE-inhibition and estrogens can be discerned. Clearly, subgroups in our study were too small to draw conclusions in this regard.
